Every taxpayer must fill out the first four pages of the IRS form 13614-C “Intake/Interview & Quality Review Sheet” prior to meeting with the volunteer tax preparer. We recommend that you fill it out at home prior to your arrival. If that is not possible, please arrive 15 minutes early to fill it out on site.

WE MUST SEE a Government issued photo ID for the taxpayer and spouse (drivers license, passport, etc)
WE MUST SEE Social Security cards for taxpayer, spouse, and dependents
(or the tax return that WE filed for you last year, or your SSA-1099, or a letter from Social Security)

Bring last year’s tax return if you have it.
Bring any tax documents that you have received for the year
If you want your refund by direct deposit bring a copy of one of your checks (no deposit slips)

If you might want to itemize deductions, you can download the Schedule A worksheet

If you operate a business, please download the Schedule C worksheet

INCOME

 SSA-1099 Social Security Year-end Earnings Statements
 All W-2s if you work
 All 1099s (e.g. 1099-R, 1099-Div, 1099-lnt, 1099-B, 1099-Misc, 1099-NEC, 1099-LTD)
 1099-G Record of state tax refund, paid family leave, or unemployment compensation
 Other income such as Gambling Winnings, K-1, or Jury Duty pay
 1099-B Brokerage statements if you sold stocks, and records of the stocks’ purchase
price if the broker’s statement does not contain that information
 Interest Year-end statements from banks
 Self-employment earnings and expenses, including cash payments and mileage

DEDUCTIONS (only if you want to itemize deductions)
 medical expenses
 charitable gifts
 real estate tax bills
 1098 mortgage interest statement
 gambling losses, etc


CREDITS / ADJUSTMENTS
 Child Care provider name, address, phone#, and tax ID (EIN or SSN) if you paid for
child care in order to work or go to school
 Student loan interest payments, education expenses, educator expenses
 Amount of any contributions to IRA accounts
 Information on a First-time Home Buyer credit from 2008 for repayment

GENERAL
 Amount of any estimated taxes that you have paid for TY2022
 Tax returns for the last two years – 2020 and 2021
 Form 1095-A if you purchased health insurance from Covered California
 FTB Form 3895 if you received a California subsidy to help pay health insurance
 Anything you receive that is labeled as an important tax document
 Banking information for direct deposit of refund
