Enter your email address and click “Email new password” in order to have a new password emailed to your account on file.

Please only press the button once. Your email may take several minutes to arrive. Be sure to check your Spam folder.

Pick a new password. Your password must be at least 8 characters long and contain at least one uppercase letter, one lowercase letter, one number, and one special character from the following list: !@#$%&*?.

Enter your new email address and click “Change email” to change the email address on your account. The new email address must not be the same as an email address already in use.

This will log you out of the appointment system. Use your new email address to log in again.

Enter your new phone number and click “Update phone number” to change the phone number associated with your account.
